recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that, employees
were exposed to being struck by a fallen load lifted by below the hook
lifting device, spreader
bar:
(a) Wall Department - On or about April 22, 2010 employees were exposed to
a struck
by hazard from falling loads, in that, the shop-made spreader bar in use
was not tested,
rated and marked.
Abatement Certification Required
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to: Test, rate and mark the load capacity of the below the hook lifting
device, spreader bar.
