Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: Well site; on or about July 22, 2009, employees were operating an oilfield service rig during the repair operation of an injection well involving the removal of well tubing, this required that the derrick man be posted on the mast mounted tubing board racking and unracking tubing; the emergency escape device (Geronimo Line) for the mast was not properly installed. The Geronimo cable had not been installed and the anchorage was inadequate rendering the Geronimo unusable. This exposed the employee to the hazards of falling and/or being caught in a rig collapse. The American Petroleum Institute (API) Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, RP 54, 6.10.1 requires than an auxiliary means of escape be installed prior to the derrick hand working on the mast. Among other methods, one feasible and acceptable abatement method to correct these hazards is to assure the Geronimo is installed in accordance with the manufacturer's instructions prior to placement of the derrick hand on the mast.
