At the worksite:  On or about January 21, 2010, and at times prior
thereto, the employer
required employees to work in the mast from the monkey board without the
emergency
escape device (Geronimo) installed per the manufacturer's instructions.
Employees were
exposed to the hazards associated with falls from heights.
Among others, one feasible and acceptable method of abatement would be to
comply
with
manufacturer's installation instructions per the owner's manual for the
emergency escape
device.
