Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm where employees were exposed to fall hazards: For employees climbing the derrick on or about January 20, 2010, and at times prior thereto, the employer did not install a tag line on the self-retracting lifeline used by the derrick hand when climbing in the derrick. The line is left extended for periods of time exceeding 24 hours which may lead to unnecessary wear and fatigue of the self-retracting device leading to failure and resulting fall hazards. Feasible abatement includes maintaining retractible fall arrest systems in accordance with the manufacturer's instructions.s
