Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: Well site; on or about May 25, 2010, employees operating an oil/gas service rig were finishing the re-completion of the well, with the derrick hand working in the mast during the removal of the well sucker rods and production tubing; the rig mast emergency escape system was not installed properly. The systems descent line was too short and its descent device was not relocated when the mast work elevation changed. These precluded the derrick hand from making timely safe emergency egress from the rig mast during rig emergencies with hazards as: fire, explosion, flying objects, and rig collapse. The emergency escape system manufacturer instructions identifies the required length of the descent line. The American Petroleum Institute Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, RP 54, 6.10.1 requires that an auxiliary means of escape be installed on the rig mast/derrick and that it provides a ready and convenient means of escape from the derrickman's working platform. Among other methods, one feasible and acceptable abatement method to limit the employee's exposure to the hazards is to install the emergency escape system in accordance with the manufacturer's instructions and industry practices.
