were causing or likely to cause death or serious physical harm to

were exposed to caught by moving conveyor belts:
a)Stairwell:  Employees who used a belt conveyor to move merchandise
between
floors 1 and 2 of the establishment were exposed to being caught by the
moving
conveyor belt.  The conveyor was not provided with stop devices at the
upper and
lower ends of the conveyor where employees are normally stationed.  On or
about
9/15/09.
Abatement Note
Among others, a feasible method to eliminate or materially reduce this
hazard is to
comply with ASME B20.1-2003, Section 6.1.2 and provide permanently
installed
pull
cords or similar stop controllers at points where employees are normally
stationed.
NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF
ABATEMENT FOR THIS ITEM.  FAILURE TO COMPLY MAY RESULT IN AN
ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
