a)  Establishment - 4-Step Ladder Stand SN 06/06:
The Leggett & Platt "Gillis" manually propelled mobile ladder stand,
missing one rubber pad
on one leg, was not removed from service exposing employees to fall hazard.
Violation observed on or about 10/5/2009.
b)  Establishment - 4-Step Ladder Stand SN 156985:
The Leggett & Platt "Gillis" manually propelled mobile ladder stand,
missing two rubber pads
on two legs and with damaged spring assembly, was not removed from service
exposing
employees to fall hazard.

c)  Establishment - 4-Step Ladder Stand SN 148929:
The Leggett & Platt "Gillis" manually propelled mobile ladder stand,
missing one rubber pad
on one leg and with damaged spring assembly, was not removed from service
exposing
employees to fall hazard.

Among other methods, feasible and acceptable abatement methods to correct
this hazard include:
1) Comply with the manufacturer's requirements and ANSI A14.7-1991 -
Safety Requirements
for Mobile Ladder Stands and Mobile Ladder Stand Platforms. Section 6.3
specifies: "Units that
are damaged or weakened from any cause shall not be used until repairs are
completed..."
Section 7.2.3 states "The owner of the unit, or employer having
supervisory control of a unit,
has the total responsibility for its proper inspection, setup, use and
maintenance."
2) Develop and implement a ladder safety procedure to ensure that those
employees accessing
a manually propelled mobile ladder stand are properly trained to set up
and use manually
propelled mobile ladder stands in accordance with the Manufacturer's
instructions. 3) Assure that
all units are inspected on a frequent basis; that they be immediately
withdrawn from service if
defect is revealed during inspection and are not used until all repairs
are completed.
