were exposed to crushing injuries:
a) On or about August 27, 2009 and times prior, employees operated
forklifts while not wearing
a seat belt exposing them to serious injuries in the event of a tip-over
or other accident.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to ensure that employees are trained regarding the hazards associated with
not wearing seat belts
and to implement an active, effective enforcement policy.
