Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, the granite slabs were not secured: a. On or about January 3, 2010, and at times prior, employees were exposed to crushing hazards from improper slab storage. Inside the shop, large slabs, weighing hundreds of pounds, were stored by leaning them against the wall. Additionally, granite slabs were stored un-secured in the storage yard and a-frame(s) used to store slabs were damaged. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that all slabs are stored properly and securely. Additionally, frequently inspect all a-frames to ensure they are not damaged. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing, or were likely to cause death or serious physical harm to employees in that employees were exposed to a potential crushing hazard: a. On or about January 3, 2010, and at times prior, the employer utilized an un-secured boom on the Daewoo powered industrial truck. The boom, manufactured by Vestil Manufacturing Corporation, was not secured to the powered industrial truck carriage. Among other methods, one feasible and acceptable abatement to correct this hazard is to follow Page 1 of the Vestil Lift Master Boom Operators Manual, which states: "Secure Lift Master Boom to fork truck carriage with safety chain". Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
