Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to falls which could causing serious injury or death: a) On or about March 10, 2010, an employee was exposed to falls exceeding 4 feet while working from a pallet that was attached to the forks of a forklift. Among other methods, a feasible and acceptable method to correct this hazard would be to fabricate a work platform with a guardrails system and follow the work procedures that are listed in ANSI B56.6.(Section 5.12). Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to falls while using the top step of a step ladder: a) On or about March 10, 2010,an employee was exposed to falls exceeding 4 feet while using the top steps of a 6 foot fiberglass step ladder to access the pallet which was attached to the forks of the forklift. Among other methods, a feasible and acceptable method to correct this hazard is to follow the requirements listed in ANSI A14.5.9.3-2000 (Section 9.3.2) for proper ladder use. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
