Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to crushed-by hazards associated with improper operation of three automotive lifts: a) On or about March 18, 2010 and at times prior, employees were exposed to crushed-by injuries while they used three hydraulic automotive lifts in an inappropriate manner as follows: 1.The safety locking mechanisms (bars) which were originally installed with the lifts were not functioning because critical pieces had been removed or broken. The safety bar was no longer connected to the lift as it rose from the floor. Without such a lock in place, there was nothing preventing the lift from dropping if hydraulic pressure was accidently or suddenly lost. 2.Vehicles were not properly contacting the lift supports and instead made contact with a block of wood which was wedged between the lift support and the vehicle. 3.Lifts did not have any posted information related to the load capacity, operating instructions or other pertinent manufacturer information. 4.Employees operating the lifts did not have the training necessary to safely operate them. Abatement: Among other methods, several feasible and acceptable abatement methods to correct the hazards would be to implement the requirements of the American National Standard for Automotive Lifts - Safety Requirements for Operation, Inspection and Maintenance (ANSI/ALI ALOIM - 1994) as well as the guidelines found in "Lifting it Right", a safety manual from the Automotive Lift Institute (ALI). These documents require that lifts be inspected to ensure, among other things, that lift locks engage in the fully extended position. Further they recommend the use of the vehicle manufacturer's recommendations with regard to vehicle contact points for the lift support. These documents also require that lift manufacturer information and ALI safety manuals are available on site and that this information is used to train employees who operate the lifts. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to stuck- by hazards associated with use of an improperly maintained garage door: a) On or about March 18, 2010 and at times prior, employees were exposed to struck-by hazards while they used a non-motorized garage door that was loose and worn and did not stay in place or stop itself from falling when employees let go of it. Employees wedged a piece of wood in the door to keep it from falling on them. Among other methods, a feasible and acceptable abatement method to correct the hazard would be to repair or replace the springs and any other parts of the door which would allow it to function as it did when it was installed or in the manner in which the manufacturer intended. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
