Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees employment or a place of employment which
were free
from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to fall hazards:
a)On or about May 19, 2010, employees were exposed to fall hazards where
the anchor
for the Geronimo line was not rated as being capable of withstanding a
pull of 3000
lbs, as recommended by the manufacturer.
The Operator's Manual for the Geronimo Emergency Escape Device states: "The
anchor to which you secure the lower end of the cable must be able to
withstand a
pull of at least 3000 pounds."
American Petroleum Institute Recommended Practice for Occupational Safety
for Oil
and Gas Well Drilling and Servicing Operations (API RP 54) states "The
ground
anchor point should be able to withstand a pull of at least 3000 lb."
b)On or about May 19, 2010, employees were exposed to fall hazards where
the wire
rope used for the Geronimo line was kinked at the top, approximately five
feet from
where it was attached to the derrick mast.
The Operator's Manual for the Geronimo Emergency Escape Device states:
"Make
sure the cable has no kinks or frayed areas."
American Petroleum Institute Recommended Practice for Occupational Safety
for Oil
and Gas Well Drilling and Servicing Operations (API 54) states that wire
rope used
for an auxiliary escape be in "good condition."
Among other methods, one feasible and acceptable method to correct this
hazardous
condition would be to anchor the escape line per manufacturer's
recommendations and to
regularly inspect the wire rope used for the escape device and replace it
as necessary.

mast was
damaged and did not close as designed.
American Petroleum Institute Recommended Practice for Occupational Safety
for Oil
and Gas Well Drilling and Servicing Operations (API RP 54), Section 5.5,
Fall
Protection, states "Manufacturer's instructions for inspection and
replacement should
be followed."
Among other methods, one feasible and acceptable method to correct this
hazardous
condition would be to replace defective or damaged fall protection
equipment as necessary.
