Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to structural collapse of the workover rig: a) On or about May 3, 2010 and at times prior, H2 Plains LLC did not ensure that employees were protected from crushing hazards due to structural collapse. Proper inspections and preventative maintenance were not being performed on the rig structure. Among other methods one feasible and acceptable abatement method to correct this hazardous condition would be to develop and implement procedures for inspection, maintenance, record keeping, and repair of rig structures that conform to API Recommended Practices 4G, Third Edition, April 2004: Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to structural collapse hazards: a) On or about May 3, 2010 and at times prior, H2 Plains LLC did not ensure that employees were protected from rig collapse hazards. 1. Metal stakes were used as guy anchors instead of a buried or twist anchor. 2. The metal stakes were not tested in the field and have no engineering rating when properly installed. 3. The chain booms used to tighten the guy lines were left on the chains, thus becoming part of the guy line. They are not rated, or approved by manufacturer for this use. Among other methods, one feasible and acceptable abatement method to correct this hazardous condition would be to install and inspect all anchors in accordance with API Recommended Practice 4G, Third Edition, April 2004 Section 14.5. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
