Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: a)On or about July 14, 2010, and at times prior, employees were exposed to a crushing hazard when working beneath a powered industrial truck, which had a boom attachment, that was not secured to ensure it did not slide off the forks. b)On or about July 16, 2010, and at times prior, employees were exposed to a crushing hazard when using a boom that was rated for 1,000 pounds to lift loads in excess of 1,000 pounds. Among other methods, one feasible and acceptable abatement method to correct Instance A of this violation is to ensure that any attachment is secured to the forklift. One feasible and acceptable abatement method to correct Instance B is to ensure that all lifting devices are used within their listed limitations. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification
