were causing or likely to cause death or serious physical harm to

were exposed to the hazard of being crushed by merchandise and forklifts
while using a dock
leveler to lower merchandise and a forklift. The MKA Hydraulic Dock
Leveler is designed only
for the purpose of providing a bridge to facilitate the loading and
unloading trailer beds:
a) Sheafe Road Customer Pick-up Warehouse, on or about 9/03/10: An employee
attempted to lower an MKA Hydraulic dock leveler loaded with a forklift
and snow
blower from the dock to a customers vehicle.
FEASIBLE AND ACCEPTABLE ABATEMENT METHODS, AMONG OTHERS,
TO CORRECT THE HAZARD ARE:
1) Follow the McGuire Hydraulic Dock Leveler AUTODOK owner's manual for
MKA Dock Leveler Models, Manufacturer's page 3 - definition and function
and
page 3 -"Safety Precautions" # 5 "Do not operate the leveler with
equipment,
material or people on the platform."
2) Follow ANSI MH30.1-2007 (American National Standard for the Safety,
Performance, and Testing of Dock Leveling Devices) Section 5 Safety
Requirements
and Considerations and 5.4 Owner Responsibility, Section 5.4.1.
