Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards:
A)Warehouse - On or about February 11, 2011, employees were not trained to
load
clothing into the pits of the two Model 160F5 Economy vertical upstroke
balers,
Machine 1 (Serial #49113) and Machine 2 (Serial #50521), by only opening
the
loading chamber doors.  The employees were loading the balers by opening
both
the loading chamber and bale chamber doors and emptying wheelbarrows into
the
pits, which were approximately ten feet deep.
Among other methods, one feasible and acceptable means of abatement is to
comply
with the requirements of ANSI Z245.5-1997 Section 7.1(c), which states
that
the
employer shall be responsible for: Establishing and following a program of
training
and instructing employees in safe methods of work before assigning them to
operate
or maintain a baler.
ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQUIRED FOR
THIS ITEM.
