furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to potential crushing hazards from falling loads and/or lift
parts:
(a) Employees were exposed to a crushing hazard while working around and
under the
Wheeltronic autolifts at 603 Second Avenue South in Hettinger, North
Dakota.
(b) Employees were exposed to a crushing hazard while working around and
under the
Forward Manufacturing Company autolifts at 603 Second Avenue South in
Hettinger,
North Dakota.
Abatement Note: Among other methods, one feasible and acceptable method of
abatement would
be to follow the requirements of ANSI/ALI ALOIM and/or ANSI B153.1 which
includes among
other requirements, the following:
1. Institute a preventive maintenance program according to the
manufacturer's
specifications.
2. Provide training to employees on proper use and care of the lifts.
3. Ensure all lifts are rated and marked with their load capacity.
4. Schedule and complete all periodic and annual inspections.
