Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees,
in that the crane
used in the scrap bin was equipped with a hoist rope which was too large
for the hoist drum,
exposing employees to potential struck-by and crushing hazards if the
hoist rope were to break.
a)Kenton Iron Products LLC, Kenton, Ohio:  On or about January 19, 2010,
the employer
did not ensure that the Series 700 Shaw-box crane, Model 72L05026S1522,
Serial No.
06I11751, used in the scrap area of the foundry, was in good repair. The
wire hoist rope
was not seated correctly in the grooves of the hoist drum due to its over
sized diameter
and exposed employees to struck-by and crushing hazards if the wire hoist
rope were to
fail and drop its load.
Among other methods, one feasible and acceptable method to correct this
hazard is to ensure that
the correct size wire hoist rope is installed and used in accordance with
ASME B30.17-2006 and
the Operation, Service & Parts Manual for the Series 700 Shaw-Box Wire
Rope
Electric Hoist.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation (
using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition,
documentation demonstrating that abatement is complete must be included
with your
certification.  This documentation may include, but is not limited to,
evidence of the purchase
or repair of the equipment, photographic or video evidence, or written
records.

steers were not in good repair as they were leaking hydraulic fluid and
safety devices such as
backup alarms were malfunctioning, exposing employees to struck-by and
crushing hazards.
a)Kenton Iron Products LLC, Kenton, Ohio:  On or about January 19, 2010,
the
employer
did not ensure that the Bobcat skid steer, Model S130, Serial #527311637
used in the
foundry to complete various tasks, was in good repair while being operated
by
employees, in that it had a hole in the chain case that was leaking
hydraulic fluid.
b)Kenton Iron Products LLC, Kenton, Ohio:  On or about January 19, 2010,
the employer
did not ensure that the Bobcat skid steer, Model S130, Serial #527311637
used in the
foundry to complete various tasks, was in good repair while being operated
by
employees, in that it was not equipped with a functioning back-up alarm.
c)Kenton Iron Products LLC, Kenton, Ohio:  On or about January 19, 2010,
the employer
did not ensure that the Bobcat skid steer, Model S185, Serial #5303151390
used in the
foundry to complete various tasks, was in good repair while being operated
by
employees, in that it  had a hole in the hydraulic holding tank that was
leaking
hydraulic
fluid.
d)Kenton Iron Products LLC, Kenton, Ohio:  On or about January 19, 2010,
the employer
did not ensure that the Bobcat skid steer, Model S185, Serial #5303151390
used in the
foundry to complete various tasks, was in good repair while being operated
by
employees, in that it was not equipped with a functioning back-up alarm.
Among other methods, one feasible and acceptable method to correct this
hazard is to ensure that
skid steers are maintained and operated according to the Association of
Equipment Manufactures
(AEM) manual "Skid Steer Safety Manual for Operating and Maintenance
Personnel" 2004
edition.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation (
using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition,
documentation demonstrating that abatement is complete must be included
with
your

did not ensure the spreader bar used at the piston molding station was
marked with its
rated load capacity, exposing employees to struck-by and crushing injuries
or death
should the spreader bar become overloaded, causing it to fail and drop its
1,000 lb load.
b)Kenton Iron Products LLC, Kenton, Ohio:  On or about January 19, 2010,
the employer
did not ensure the spreader bar used at the Rotolift molding station was
marked with its
rated load capacity, exposing employees to struck-by and crushing injuries
or death
should the spreader bar become overloaded, causing it to fail and drop its
900 lb load.
Among other methods, one feasible and acceptable method to correct this
hazard is to ensure the
under the hook lifting devices are labeled according to ANSI B30.20 Below
the Hook Lifting
Devices.
In accordance with 29 CFR 1903.19(d), abatement certification is required

the employer did
not ensure that the Series 700 Shaw-box crane, Model 72L05026S1522, Serial
No. 06I11751,
used in the scrap bin area of the foundry, was being properly operated by
employees, in that the
crane was being used for side pulls.  The employer had replaced multiple
damaged
wire hoist
ropes as a result of the side pulls and exposed employees to struck-by and
crushing hazards if
the wire hoist rope were to fail and drop its load while being used in
this manner.
Among other methods, one feasible and acceptable method to correct this
hazard is to ensure the
crane is maintained and operated according to the ASME B30.17-2006
"Overhead and Gantry
Cranes".
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation (

employees, in that foundry
employees working with Tube Shake-out and Shake-out cranes were exposed to
the potential
hazards of being struck-by or crushed by materials carried by the cranes
if the materials were
to be disengaged from the crane hooks due to missing safety latches.
a)Kenton Iron Products LLC, Kenton, Ohio:  On or about April 28, 2010, the
employer
did not ensure that the hook for the Shake-out crane, a CM-2 Ton crane,
located in the
foundry area was equipped with a safety latch, exposing employees to
struck-by and
crushing hazards should the object being lifted roll out of the hook.
b)Kenton Iron Products LLC, Kenton, Ohio:  On or about April 28, 2010, the
employer
did not ensure that the hook for the Tube Shake-out crane, a Budgit
manufactured Series
6000, Model BAH 0130, located in the foundry area was equipped with a
properly
working safety latch, exposing employees to struck-by and crushing hazards
should the
object being lifted roll out of the hook.
Among other methods, one feasible and acceptable method to correct this
hazard is to comply
with the requirements of ASME B30.10-2009 "HOOKS Safety Standard for
Cableways, Cranes,
Derricks, Hoists, Jacks, and Slings" and ASME B30.16-2007 "Overhead Hoists
(Underhung)".
This employer has received a citation for this standard or a substantially
similar standard
as reflected in inspection #311604938, Citation #02, Item #01, issued on
January 23, 2009
and became a final order on March 10, 2009.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation (
