Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that employees
were exposed to crushing hazards while moving dies:
a)  L&W Engineering Co. - Avon, Ohio: On or about May 21, 2010, the
employer failed to
assure die handling procedures that would prevent a metal support plate
from falling during die
lifting operations.  Employees who perform die changes for the 1200 ton
Minster press were
exposed to crushing hazards in that the metal support plate used to
transfer dies was not secured
when employees used an overhead crane to lift press dies off of the metal
plate onto the transfer
cart.  The metal plate could roll or fall from the die staging table and
crush employee(s) that
handle press dies.
Abatement Note: Among other methods, one acceptable and feasible means of
abatement
is to
secure or block the metal support plate from rolling/falling off of
staging tables.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
( using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence, or
written records.
