Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards associated with being crushed by or struck by
falling objects:
Drive Thru Warehouse (In & Outdoor Racks) and Building #3 Outdoor Racks:
On or about September 22, 2009, the storage rack systems used to store
lumber, roofing
shingles and other building materials were not installed so as to prevent
damage or
collapse of the racks.  Deficiencies included, but were not limited to:
a) Collision protection for the bottom portions of the frames.
b) Anchoring of columns.
c) Use of permanent plaques placed in the environs of the existing racks
designating load sizes and other important information.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to:
a) Provide column protectors and/or steel clad impact panels.
b) Provide anchors that are capable of resisting sheer forces caused by
horizontal and
vertical loads.
c) Users should consult with the manufacturer to supply the applicable
plaques or obtain
a predetermination by a qualified person and the rack loaded in accordance
with their
requirements, plaques would not be necessary.ts,
