Section 5(a)(1) of the Occupational Safety & Health of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employer did not ensure that each attendant remained outside the confined space at all times during entry operations until relieved by another attendant: Location:: Steam Tunnel, 243 Charles Street, Boston, MA On 12/20/2009, the employees servicing the "New Steam Service" system were not trained on the emergency evacuation exit route within the steam tunnel.
