An employee using a rope descent system did not tie back his life line to
an independent
anchorage as per ANSI IWCA I-14.1 2001: section 9.1.3 and 14.6.2.
Among other methods the following are feasible and acceptable means of
abating the hazards:
1. Comply with ANSI IWCA I-14.1 2001 Window Cleaning Safety Section 5.7.2
which
requires employees to be trained in identifying anchorages for both a
support system and
personal fall protection systems.
2. Comply with ANSI IWCA I-14.1 2001 Window Cleaning Safety Section 5.6.7
which
requires that rope descent systems be eqquipped with and utilize an
independent fall arrest
system complying with Section 3.8 and Section 9.2.2.
3. Submit a plan of service to the building owner, or his agent which
addresses safety
concerns and identifies the use of all anchorage points prior to the
service
being performed.
4. Perform an evaluation to ensure the employees have full comprehension
of the
company safety training.
