Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards associated with improper set up and use of
ladders:
Tempering Room, Fish Inspection Lab:  On 7-7-10 a cleaning crew worker was
climbing a
ladder over the tempering room inspection lab while holding a bucket of
water. The worker fell
from the seventh or eight rung to the floor below.  The worker suffered
injuries to his head,
jaw, and shoulder.
Methods of abatement include but are not limited to:
1. Evaluate the job beforehand to determine whether a ladder can be used
for the
application.  Provide an alternate to using the ladder for example
scissors lift or aerial
lift for cleaning and painting the walls above the inspection lab.
Evaluate
hand carried
loads to ensure safety and stability;
2. Provide training to workers on the safe use of ladders including but
not limited to:
keeping three point contacts while climbing ladders, ensuring that the
ladders are set up
and secured safely, calculating the size of ladder needed and ensuring
that ladders extend
three feet above the landing;
