serious physical harm to employees in
that employees were exposed to fire hazards:
a)nursing home-  on or about 3/3/2010, a 20 pound propane gas cylinder
(Rapid X-Change) was
being used with an attached infrared tank top burner within the front
hallway to provide heat
for residents and staff.
Abatement Note:   Among other methods, one feasible and acceptable
abatement method to correct this
hazard is to follow the manufactures, Rapid X-Change, recommendations for
the propane gas cylinder use.
The cylinder was designed to be used with gas grills.d
