North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion hazards: a)Mill Shop - where employees were using equipment such as, planers and radial saws which had flexible hoses attached to the exhaust system which were not bonded to the equipment and metal dusts. ABATEMENT NOTE: a)One acceptable abatement method to correct this hazard is to bond all flexible hoses to the equipment and metal dusts, they are attached too. Reference a)Hazards are recognized and addressed in NFPA 664 paragraph 7.9.2; "Where equipment is subject to the accumulation of static charge, the accumulation of static electric charge shall be controlled by one of the following: (1) Permanent grounding and bonding of production equipment. (2) Grounded metal combs to provide discharge paths. (3) Other means shown to be effective and acceptable to the authority having jurisdiction." 0 0 y 0 4

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion hazard: a)Mill Shop - where the employer did not have a housekeeping program to prevent fire and dust hazards within the facility. ABATEMENT NOTE: a)One acceptable abatement method is to correct this hazard is to develop and implement a housekeeping program to control and prevent fire and dust hazards at the facility, such as documented housekeeping and inspection programs, regular cleaning frequencies established for walls, floors, and horizontal surfaces, such as equipment, ducts, pipes, hoods, ledges, beams, and above suspended ceilings and other concealed surfaces, to minimize dust accumulations within operating areas of the facility. Reference a)Hazards are recognized and addressed in NFPA 664 paragraph 11.1.3; "Documented housekeeping and inspection programs shall be developed and maintained."

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion hazard: a)Mill Shop - where the employer did not develop and implement an inspection, testing and maintenance program to prevent fire and dust explosions within the facility. ABATEMENT NOTE: a)One acceptable abatement method to correct this hazard is to develop and implement a program to prevent fire and dust explosions within the facility to include; 1) fire and explosion protection and prevention equipment in accordance with the applicable NFPA standards, 2) Dust control equipment, 3) Documented housekeeping and inspection programs, regular cleaning frequencies established for walls, floors, and horizontal surfaces, such as equipment, ducts, pipes, hoods, ledges, beams, and above suspended ceilings and other concealed surfaces, to minimize dust accumulations within operating areas of the facility, 4) Potential ignition sources, 5) Electrical, process, and mechanical equipment, including process interlocks, 6) Process changes, 7) Lubrication of bearings. Reference a):Hazards are recognized and addressed in NFPA 654 paragraph 12.1.1; "An inspection, testing, and maintenance program shall be developed and implemented to ensure that the fire and explosion protection systems and related process controls and equipment perform as designed."
