40.1-51.1(a) of the Code of Virginia: The employer, Shibuya Hoppmann Corporation, did not furnish a place of employment that was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were operating a hydraulic press brake where the foot treadle was not guarded to prevent accidental actuation, In the Welding Area, a Di-Acro Hydraulic Press Brake, (serial number JH1031), is operated by employee 4 hours per week. No cover was provided for the foot treadle on the press brake to prevent accidental actuation by falling or moving objects, or someone accidently stepping on it. One feasible and acceptable abatement method, among other methods, to correct this hazard would be to provide a cover over the foot treadle for the press brake such as that called for by ANSI B11.3-1982, American National Standard Safety Code for Press Brakes.
