a.At the Elvis Cirque Du Soleil Theater, a factory worker was severely
injured
when he fell 27 feet from the top of a theater stage into a stage lift
shaft.  The
C5, a stage lift had been lowered to the basement to in order to place show
elements on it.  The employer had instituted a procedure for moving show
elements.  The procedure did not adequately address protecting employees
from
falling into open stage lift shafts.
One feasible means of abatement, among others, is to update the current
procedure
for clearing
and moving show elements to include methods to protect employees from
falling into open stage
lift shafts.
