Nevada Revised Statute 618.375(1):  Duties of employers.  The employer
failed to furnish
employment and a place of employment which was free from recognized
hazards that were
causing or likely to cause death or serious physical harm to his employees:
The employer failed to ensure that a wire rope static line used for fall
protection was properly
secured with wire rope clips, as follows:
a.At the Hoover Dam Bypass Bridge construction site, employees were
exposed to
a fall hazard of 890 feet while tied off to a static lifeline that was not
anchored
correctly in that the wire rope clips were not installed in accordance
with the
manufacturer's specifications, as follows:
Three wire rope clips were being used to secure the north end of the static
lifeline around an anchor. The saddle portion of the clips were observed
to be installed onto the dead end of the wire rope with the U-Bolt portion
on the live end. Furthermore, the wire rope clips had been tightened with
a ratchet wrench and socket rather than a torque wrench as was required
by the manufacturer.
Two employees tied off to this lifeline, in the condition as described
above, to
perform the abatement of a loose top rail that was part of an adjacent
guardrail
system, which had a deflection less than 39 inches above the
walking/working
surface.

*Failure to read, understand, and follow these instructions may cause death

*Read and understand these instructions before using clips.
*Match the same size clip to the same size wire rope.
*Prepare wire rope end termination only as instructed.
*Do not use with plastic coated wire rope.
*Apply first load to test the assembly. This load should be of equal or
greater weight than loads expected in use. Next, check and re-tighten nuts
to recommended torque (See Table 1).
1.Refer to Table 1 in following these instructions. Turn back specified
amount of rope from thimble or loop. Apply first clip one base width
from dead end of rope. Apply U-Bolt over dead end of wire rope - live
end rests in saddle (Never saddle a dead horse!). Use torque wrench
to tighten evenly, alternate from one nut to the other until reaching
the recommended torque.
One feasible means of abatement would be to require employees assigned to
install and/or tie off onto static lines to successfully complete a
refresher training
course that emphasizes the requirements specified by the manufacturer.
