Nevada Revised Statute 618.375(1): The employer failed to furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees: The employer failed to ensure the hydraulic shear operator was trained, as follows: a.At the Expo Promotions, Inc. printing warehouse, hydraulic shear operations, Employee #1 was exposed to hazards while operating a hydraulic shearing machine without being trained. Reference: ANSI (American National Standard Institute) B11.4-1993 for Machine Tools - Shears - Safety Requirements for Construction, Care, and Use Paragraph 8.2 Training: The employer shall instruct operators in the proper use of [the] shear production system. One feasible means of abatement is to provide operator training that would include a description of the assigned task, the function of controls to be encountered in performing the assigned task, the hazards associated with the assigned task, the designated method of feeding, the designated safeguarding, and the methods of function-testing or otherwise assuring the proper function of safeguarding.
