Nevada Revised Statute 618.375(1): Every employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or were likely to cause death or serious physical harm to his employees: The employer did not furnish employment and places of employment which was free from recognized hazards that were likely to cause death or serious harm to his employees, as follows: a) In the distribution center, the employer allowed trained employees to go under the Dematic conveyor system to unjam the conveyor system while it was still running. On June 9, 2010, an employee was injured when her hair got caught in the conveyor system as she went under the conveyor to clear the jam. Per the Operator's Manual, January 1, 2006: Page 2: "Do NOT perform service or maintenance until all power is locked out or disconnected." Page 3: "The regulations defines servicing and maintenance as covering "lubrication, cleaning or unjamming of machines or equipment...where the employee many be exposed to the unexpected energization or start up of equipment.." among other things." ** Page 13, Item #2: "Turn off and lockout the power to the conveyor before clearing the jam". A feasible means of abatement would be to follow the Operator's Manual and lockout the power to the conveyor before clearing the jam.

Nevada Revised Statute 618.375(1): Every employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or were likely to cause death or serious physical harm to his employees: The employer did not furnish employment and places of employment which was free from recognized hazards that were likely to cause death or serious harm to his employees, as follows: a) In the distribution center, the employer allowed trained employees to go under the Dematic conveyor system to unjam the conveyor system by using the emergency cable to stop the machine. Per the Operator's Manual, January 1, 2006: Page 2: "Do NOT perform service or maintenance until all power is locked out or disconnected." Page 3: "The regulations defines servicing and maintenance as covering "lubrication, cleaning or unjamming of machines or equipment...where the employee many be exposed to the unexpected energization or start up of equipment.." among other things." ** Page 7: "E-stop devices are not intended to disable equipment to provide protection for personnel in potentially hazardous contact with such equipment. FOR EXAMPLE, MAINTENANCE, JAM CLEARING AND/OR OTHER SIMILAR ACTIVITIES REQUIRE ADHERENCE TO LOCKOUT PROCEDURES." Page 13, Item #2: "Turn off and lockout the power to the conveyor before clearing the jam". A feasible means of abatement would be to follow the Operator's Manual and lockout the conveyor system while jam clearing.
