Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls from elevated surfaces: a) On or about 01/29/10 at 161 Helen Street, South Plainfield, NJ 07080. Warehouse, Hallway from side entrance: Employees raised while standing on a wood pallet on the forks of a Komatsu 25 Fork Lift Truck to remove pipes and close off valves were not protected from the hazard of falling to the lower concrete floor. Employees worked from heights of approximately 20'. Abatement Note: One feasible and acceptable abatement method, among others to correct this hazard, is to provide a "Safety Platform" properly secured to the lifting carriage and/or forks per ANSI B56.1-1975, Part III - Section 513, or equivalent protection.
