Floor and
Ground Level: "Freight Elevator"
a/  The gate to the "Freight Elevator" descending from the second floor to
the ground
level was not provided with a interlock (limit switch) to prevent the
elevator from being
operated with the gate/door in the open position.
b/  Ground Level - Waste Container Area Below Warehouse - The gate/door to
the
"Freight Elevator" ascending was not provided with a interlock (limit
switch) to prevent
the elevator from being operated with the gate in the open position.
One feasible and acceptable abatement method, among other methods, to
correct this
hazard is to:
1/  Provide interlocks (limit switches) on the elevator gates/doors at the
second floor
level and ground level to prevent the elevator from moving when the
gate/door is in the
open position.
a/  The interlock device (limit switch) shall hold the gate/door in the
locked position by
means of gravity or by a restrained compression spring, or by both, and/or
by means of
a positive linkage.
b/  The interlock device (limit switch) shall lock the door in the closed
position before
the elevator can be operated.  When the interlock (limit switch) is
released, the elevator
is to be rendered inoperable.
