to cause death or serious physical harm to employees: (a)Date of Violation:12/09/09 Location:Fabrication Area Equipment:Belfab Dust Collector, Model: #NBMOP, Serial: #2004-08-18192-1 Condition:Employees were exposed to potential dust explosion and fire hazards from the ignition of wood dust inside an enclosureless collector that was, among other things, operating at 6,000 cubic feet per minute, located inside approximately 4 feet from the nearest emergency exit, and emptied of dust contents approximately once every week. (b)Date of Violation:12/09/09 Location:Fabrication Area Equipment:Dantherm Filtration S Bag Filter Dust Collector, Model: #S-750, Serial: #DVK213TTFW43408A-L Condition:Employees were exposed to potential dust explosion and fire hazards from the ignition of wood dust inside an enclosureless collector that was, among other things, located inside directly adjacent to an employee work station and emptied of dust contents approximately once every week. Among other methods, one feasible and acceptable means of abatement would be to comply with guidelines outlined in the National Fire Protection Association (NFPA) 664 Standard for the Prevention of Fires and Explosions in Wood Processing and Woodworking Facilities. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
