a)At lot behind auditorium building: On or about February 9, 2010 and at

prior, the employer exposed employees to struck-by hazards, in that the
missing
hook latch was not in place to prevent suspended loads from slipping off
the hook
when employees operate the crane to land bundles of roofing panels on the
roof
of the building.
Feasible abatement methods include, but are not limited to: install the
hook latch;
requiring employees to conduct and document pre-operation inspections to
identify
deficiencies; Have semi-annual and annual certifications of the crane by a
qualified
party.
