Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)  For the employees exposed to a crushing hazard on the Bendpak
automotive lift that
was lifting a vehicle chassis with three arms that were not properly
secured, located in
the shop area at 202 First Avenue East, Williston, ND.
(b)  For the employees exposed to crushing hazards on a Challenger lift
that was not
properly secured with four mounting bolts on the internal flanges located
in the shop area
at 202 First Avenue East, Williston, ND.
Abatement Note:
Among other methods, one feasible and acceptable method would follow the
requirements
of ANSI/ALI ALOIM and/or ANSI B153.1 which include, among other
requirements,
the following:
1.  Proper installation and mounting in accordance with manufacturers
specifications.
2.  Operator training on proper lift usage in accordance with
manufacturers specifications
3.  Institute a preventative maintenance program according to manufacturers
specifications.
4.  Schedule and complete all periodic inspections.
