Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or physical harm to employees in
that employees were
exposed to:

El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
At the second floor of "Administracion de Servicios Medicos de Puerto
Rico, Hospital
de Trauma Unidad Intermedio I", it was observed an emergency door in front
of nursing
counter that had a system of alarm with magneto that was not functioning.
The employer
utilized those systems in doors for the control of unauthorized people and
to offer
security to employees, patients and family due to that high-risk patients
were attended
such as people involved in robberies, murders, etc. During the inspection
could be
certified that from time to time unauthorized people access the place
without any type of
control. In August 27, 2009 there was an incident where a person utilizing
another  way
of access, entered to the place and murderer a patient. The employees
performed nursing
duties such as attending patients, to administer medicines among others.
Hazard: Fatalities, injuries, and illnesses related to violence in the
workplace.
Method to abate the hazard: The employer shall provide maintenance to the
emergency
doors and its security features to maintain safe workplace to employees.
En el segundo piso de la Administracion de Servicios Medicos de Puerto
Rico, Hospital
de Trauma Unidad Intermedio I, se observo la puerta de emergencia frente
al mostrador
de enfermeria que tenia un sistema de alarma con magneto que no estaba
funcionando.
El patrono utilizaba esos sistemas en las puertas para el control de
personas no
autorizadas y para ofrecer seguridad a empleados, pacientes y familiares
debido a que
se atendian pacientes de alto riesgo como personas involucradas en robos,
asesinatos, etc.
Durante la inspeccion se pudo certificar que en ocaciones personas no
autorizadas
accedian al lugar sin ningun tipo de control. En agosto 27 de 2009 hubo un
incidente
donde una persona utilizando otra manera de acceso, entro al lugar y
asesino a un
paciente. Los empleados realizaban tareas de enfermeria como atender
pacientes,
administrar medicinas, entre otros.
Riesgo: Fatalidades, lesiones, y enfermedades relacionadas a la violencia
en el sitio de
trabajo.
Metodo para corregir el riesgo: El patrono tiene que proveer mantenimiento
a las puertas
de emergencia y sus componentes de seguridad para mantener un sitio de
trabajo seguro
a los empleados.
