recognized hazards that
were causing or likely to cause death or physical harm to employees in
that employees were
exposed to:
Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico:
El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
At the warehouse of Guaynabo NL Stores Inc. DBA National Lumber & Hardware;
metal shelves of 2 levels, stacked one over the other and they had 4 feet
of heigh each.
Crystal blocks were stored inside these shelves. The shelves we observed
with bent legs
and presented deflection in the bases where located the merchandise boxes.
In this area
the employees received, stored and dispatched hardware merchandise.
Hazard: Falling of the shelves and/or the merchandise above the employees.
Among other correction methods, a feasible and acceptable was repair or
substitute the
shelves.
En el almacen de NL Stores Inc. DBA National Lumber & Hardware de Guaynabo;
anaqueles de metal de 2 niveles, colocadas uno sobre el otro y tenian 4
pies de altura
cada uno. Dentro de los anaqueles habian bloques de cristal almacenados y
se
observo
que los anaqueles tenian las patas dobladas y  presentaban defleccion en
las bases donde
se encontraban ubicadas las cajas de mercancia. En esa area los empleados
recibian,
almacenaban y despachaban mercancia de ferreteria.
Riesgo: Caida de los anaqueles y/o la mercancia encima de los empleados.
Entre otros metodos de correccion uno factible y aceptable seria reparar o
sustituir los
anaqueles.

At the warehouse of Guaynabo NL Stores Inc. DBA National Lumber &
Hardware; ;
metal shelves stacked up to 4 levels, one over the other and had 3 feet
clearance betwen
shelves, they contained hardware merchandise, were not bolted to the
floor. The
employees moved, stored and shipped merchandise from the shelvess by hand
and/or
using wheels carts,and a diesel forklift, trademark Hyster, model H 60
XL-MIL, serial
number B 177G2833L, as was the case. The shelves designed to be bolted to
the floor,
they had holes for screws.
Hazard: Falling of the shelves and/or the merchandise above the employees.
Among other correction methods, a feasible and acceptable was anchor the
shelves to the
floor.
En el almacen de NL Stores Inc. DBA National Lumber & Hardware de Guaynabo;
anaqueles de metal de hasta 4 niveles, colocados uno sobre otro y tenian 3
pies de altura
cada uno. Dentro de los anaqueles habian bloques de cristal almacenados y
se observ'o
que los anaqueles contenian mercancia de ferreteria no estaban
atornillados al piso. Los
empleados movian, despachaban y almacenaban mercancia de los anaqueles de
forma
manual y/o utilizando carros de ruedas o un montacarga marca de diesel,
marca
"Hyster", modelo H 60 XL-MIL, numero de serie B 177G2833L;. Los anaqueles
estaban
disenados para estar atornillados al piso.
Riesgo: Caida de los anaqueles y/o la mercancia encima de los empleados.
Entre otros metodos de correccion uno factible y aceptable seria anclar
los anaqueles al
