Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling hazards due to the climbing assist from the drill rig floor to the monkey board due to a) wire rope was not installed with a thimble, and b) wire rope clips were not properly installed. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS: Install a thimble on any wire rope connection and properly space wire rope clips in accordance with wire rope manufacture. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.

Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards; Employees working drilling rig # 2 at Schlochtermeier Well # 5, in Ellinwood, KS were exposed to fire and explosion hazards created in that the auxiliary escape line was not set up for employees to quickly evacuate the derrick platform in the event of an emergency. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS: Install the auxiliary escape line route prior to any drilling operations. The employer should use a specially rigged and securely anchored escape line attached to the derrick mast so as to provide a ready and convenient means of escape for the derrickman's working platform as referenced in paragraph 6.10.1 of API RP 54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, 1999 edition. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.
