Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards: Employees working on the derrick platform were exposed to fire and explosion hazards created in that the auxiliary escape line was not set up for employees to quickly evacuate the derrick platform in the event of an emergency. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS: Install the auxiliary escape line route as soon as an employee begins working from the derrick platform. The employer should use a specially rigged and securely anchored escape line attached to the derrick or mast so as to provide a ready and convenient means of escape for th derrickman's working platform as referenced in paragraph 6.10.1 of API 54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, 1999 Edition. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.

Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by or falling object hazards: The wire rope used to support the drilling tongs was not installed with enough wire rope clips for the size of wire rope used. Two wire rope clips were used on the 5/8 inch wire rope. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS: Install a minimum of three wire rope clips when the size of the wire rope is greater than 7/16 inch as recommended in Chapter 4 of the Wire Rope Users Manual from the Wire Rope Technical Board. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.
