29 CFR 1910.119(f)(1)(iii): The employer's written operating procedures did not address the requirements for the safety and health considerations listed in 29 CFR 1910.119(f)(1)(iii)(A) through (f)(1)(iii)(E): Employees changing out one-ton chlorine cylinders were exposed to chlorine hazards in that the operating procedure did not address adequately the types of personal protective equipment needed to protect the employees and the actions needed if those precautions failed. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.

29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process: At the facility, the employer did not implement or develop a written management of change to identify and evaluate all scenarios that might result in the release of toxic chlorine vapors when changes to the technology, equipment, and procedures were made that impacted the process. Including, but not limited to the following: (a)Changing the operator's personal protective equipment from a SCBA to a 5 minute escape pack while changing ton cylinders. (b)Removing the hand held radios from the ton cylinder standard operating procedure. (c)Replacing a black flexible hose that was attached to the automatic switchover to the inlet nipple of the chlorinator with a Clearflo clear vinyl flexible hose. (d)Disconnecting the high and low vacuum switches on the chlorinators. (e)Adding a 2 inch water pipe line bypass for the 3 inch water inlet to the chlorine building. (f)Replacing a Capital Controls Company chlorine gas detector with a MSA chlorine gas detector in the chlorine building. (g)Painting the hard pvc piping that is transferring chlorine gas vapors. (h)Removing the gas & liquid ton container manifold system that accommodated 4, ton cylinders to a 2, ton cylinders connected directly to individual vacuum regulators. Employee(s) were exposed to release of chlorine compounds to the atmosphere that can lead to death or injury to persons working in the chlorine building. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace: Employees working in the water treatment plant were exposed to hazards associated with respirator use in that medical evaluation were not completed prior to employees being authorized to use respirators. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
