29 CFR 5.a(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards. a.On or about January 24, 2011, an employee was operating a rotary lift (model SPOA-200) to lift a Ford Expedition. This lift had worn gears. Additionally, this rotary lift was not periodically inspected. Among other feasible means of abatement is to follow the safety and instructions in the manufacturer's "Safety Manual from the Automotive Lift Institute" or The American National Standard ANSI/ALI ALOIM: 2008 "Standard for Automotive Lifts - Safety Requirements for Operation, Inspection and Maintenance."
