a) Brake Department -
Employees were exposed to an amputation hazard while operating unguarded
press brakes. As
an alternative to guarding, the employer had not developed and implemented
a safe distance
program. On or about 03/25/2010.
NOTE: One feasible abatement method would be to develop and implement a
safe distance
program that complies with the requirements of ANSI B11.3-1982 and OSHA
Directive CPL
2-1.25.
NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS
REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE
TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN
ACCORDANCE WITH 29 CFR 1903.19.
