General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to crushing hazards associated with failing to ensure that under slung chain hoists were inspected and in good repair as outlined in the manufactures data. Failure to inspect the hoist can lead to the unit failing and unexpectedly dropping the lifted load. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: (a) Develop and implement written procedures requiring the daily, monthly, and annual inspection of hoists at the facility. (b) Develop and implement a disciplinary action program and procedures to ensure that employees perform the inspections as noted in item (a) above. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.
