Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees are exposed to crushing and amputation hazards. Maintenance bays in the facility located at 3427 State St., Grand Island, NE - The employer is failing to protect employees utilizing hydraulic automobile lifts from crushing and amputation hazards due to the safety locking devices not working properly. Graham Tire Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 5(a)1, which was contained in OSHA inspection number 312676026, Citation #1, Item #1, issued on 1/6/2009 and became a final order on 1/28/2009. Among other methods, the most feasible and acceptable method of abatement is to follow the requirements of ANSI/ALI ALOIM and/or ANSI B153.1 which includes, among other requirements, the following: 1) Develop, implement and strictly enforce a preventive maintenance program according to the manufacturer's specifications. 2) Provide training to employees on proper use and care of the lifts. 3) Ensure all lifts are rated and marked with their load capacity. 4) Schedule and complete all periodic and annual inspections. 5) Increase scheduled inspection frequency from quarterly to weekly and ensure all are conducted and documented. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
