practices (RAGAGEP).
Unlabeled and/or improperly labeled valves, pressure relief valves, and
piping
including, but not limited to RV #179 and RV #237 did not comply with
RAGAGEP such as International Institute of Ammonia Refrigeration (IIAR)
Bulletin #114 and facility mechanical integrity program.

replaced at least every five years to comply with RAGAGEP such ASHRAE-15
and the facility mechanical integrity program.

Universal Cold Storage & Trucking, LLC - Lincoln, NE - The employer did
not correct
deficiencies in equipment that were outside acceptable limits (as defined
by the process safety
information in paragraph (d) of this section) before further use or in a
safe and timely manner.
A) Emergency exhaust of 11,147 cfm in south engine room does not meet the
minimum requirements per IIAR-2-1999 and company mechanical integrity
program inspection checklist.
B) Relief discharge piping on equipment including, but not limited to the
economizer (EC-HSC-3) and the surge vessel (SV-3 for AU-37) is too small
and
length exceeds allowable length per 1992, 2000 relief piping code and
facility
inspection tracking report.
