29 CFR 1910.119(e)(3)(v): The employer did not ensure that the process hazard analysis included the identification of all facility siting which would have a likely potential for catastrophic consequences in the workplace: 555 South Stuhr Road, Grand Island, NE 68801 (Engine Room): The employer did not ensure that process hazard analysis included the identification of prevention of impact or some sort of barrier control to protect refrigeration equipment such as, lines, vessels, or pumps from vehicular impacts, which would have a likely potential for catastrophic consequences in the workplace. In the engine room, there are ammonia vessels, including, but not limited to, the HSSC-7 vessel that are exposed to forklift impacts, as there are no barriers or other means to prevent impact. This information was not included in the most recent process hazard analysis (PHA), which was completed in 2009. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.ion
