Tyson Fresh Meats, Inc. - Lexington, NE - The frequency of inspections and
tests of process
equipment to maintain its mechanical integrity, was not consistent with
applicable manufacturers'
recommendations and good engineering practices, or more frequently
determined to be necessary
by prior operating experience.
Pressure/system relief valves for process equipment including, but not
limited to
ammonia compressors, pressure vessels and associated piping, had not been
tested
and/or replaced with the frequency needed to comply with recognized and
generally accepted good engineering practices such as IIAR Bulletin 109.
Process
pressure/system relief valve testing/replacement also did not comply with
the
facility's PHA and mechanical integrity program 5-year replacement
schedule.

Appropriate checks and inspections were not performed to assure that
equipment was installed
properly and consistent with design specifications and the manufacturer's
instructions.
Employee(s) installed process valves in locations including, but not
limited to the
Hot Box and freezer roof, without following manufacturer's torquing
requirements. Employer failed to verify valves were installed properly,
according
to manufacturers's recommendations per ANSI/IIAR 2-2008 Section 10.3.1.2.

that the equipment
in the process complied with recognized and generally accepted good
engineering practices.
The co-location of gas-fired boilers that use combustion air from the
engine/machinery room and refrigerant-containing equipment including, but
not
limited to compressors, does not comply with ANSI/ASHRAE 15-2004 - 8.11.6.
Abatement certification and abatement documentation is required for this
violation. The
documentation should include written verification of abatement, applicable
measurements or
monitoring results, and photographs or videos which you believe will be
helpful. The abatement
certification sheet is enclosed with the citations.
