Road, West Point, NE 68788 (Jobsite): The employer did not develop or implement written operation procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and addressed at least the following elements included in this subpart including, but not limited to; emergency shutdown as required by subparagraph (E). The process hazard analysis (PHA) was completed July 2008. However, the PHA did not have any procedures available for overriding or by-passing equipment at the facility. This includes, but is not limited to; written procedures for controlling situations where safety mechanisms might be overridden. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.
