recognized
hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard(s) of being ejected from the
operator's compartment and
being crushed by a forklift:
a)Throughout the facility, on or about August 9, 2010 and at times prior
to,
employees did not wear seatbelts while operating the forklifts, resulting
in a
crush-by hazard.
ABATEMENT CERTIFICATION AND DOCUMENTATION ARE
REQUIRED.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to ensure that employees wear a seatbelt while operating a powered
industrial truck, as required
by ANSI B56.1.ed

hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard(s) of a dust explosion, deflagration,
and fire hazards
resulting from inadequate combustible dust control:
a)In the production area, on or about August 9, 2010 and at times prior
to, the dust
collection systems for the blanket machines and the wattle machine did not
prevent the escape of Class II combustible straw dust, which was cleaned
off the
process equipment with compressed air without controlling ignition sources.
ABATEMENT CERTIFICATION AND DOCUMENTATION ARE
REQUIRED.
Among other methods, feasible and acceptable abatement methods to correct
this hazard are to
1) ensure that the dust collection systems prevent dust escaping the
process equipment, and 2)
ensure that all ignition sources are removed prior to using compressed air
for cleaning surfaces
in accordance with NFPA 61, "Standard for the Prevention of Fires and Dust
Explosions in
Agricultural and Food Processing Facilities", 2008 edition.es",
