that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
a)   On or about September 15, 2010, an employee not wearing a seatbelt
while operating
a Genie Telescopic GTH-636 powered industrial truck, was exposed to being
thrown or
ejected from the equipment and/or crushing hazards.
Among other methods, one feasible and acceptable method to correct the
hazard is to
require and enforce the use of the serviceable seatbelt that was mounted
on the equipment
and per the requirement in the Genie Telescopic GTH-636 operator's manual.
